Q. What is Long Term Care?

A. Long Term Care includes a variety of health and other services used by people of any age who need help to function in their daily lives. Long term care contains a mix of medical and non-medical services that can be provided in various settings. This may include care provided by family members, early childhood intervention, home-delivered meals, adult day care, case management, assisted living, personal care services, nursing, physical therapy and nursing home care.

A. Anyone can make a referral for themselves or someone else.

A. There is no charge for the assessment or case management services.

A. Any resident of Fulton County can receive an evaluation for services.

A. Usually an evaluation is done within an hour.

A. The Central Assessment team provides a complete social, financial and health assessment. Any information regarding your health and financial status would be helpful.
